InTake Coordinator

Hours: Mon-Thurs 8:30-5:30, flex Fri 8:30-4:30

JOB SUMMARY:

Responsible for meeting with clients placed at Short-Term Residential Therapeutic Program to gather information to complete psychosocial assessments, intake summary and treatment planning. Responsible for communication with county social workers and probation officers regarding youth’s history and current needs.

 

ESSENTIAL JOB DUTIES:

 

  1. Responsible for intake responsibilities and completing all necessary admission paperwork.
  2. Meet with client, in home, school, field, or office to gather needed information.
  3. Complete Intake Paperwork in accordance with Department of Mental Health standards.
  4. Conduct psycho-social assessments and treatment planning in accordance with Department of Mental Health standards and agency standards.
  5. Utilize DSM and ICD-10 for diagnosing.
  6. Develop and create goals and objectives in collaboration with client and client’s Child and Family Team.
  7. Collaborate with treatment team to ensure quality client care.
  8. Coordinate linkage to appropriate intra-agency and community resources, including coordination of client’s psychiatric needs.
  9. Interface with Department of Children and Family Services, Department of Mental Health, Department of Probation, court systems, schools, law enforcement, health care providers and other community liaisons.
  10. Participate as a member of a multi-disciplinary treatment team providing critical clinical input related to client’s mental health and recommended treatment plans
  11. Participates in case conferences if needed
  12. Demonstrate empathy to work with children/families under stress
  13. Maintain confidentiality as required by St. Anne’s, licensing, HIPAA, and other regulatory agencies.

QUALIFICATIONS:

  1. Master’s Degree in Social Work, Psychology or Counseling is required.
  2. Registered with the California Board of Behavioral Sciences is required.
  3. Good organizational and communication skills as well as a strong time management.
  4. Skilled in engaging, motivating and working with diverse individuals and groups to accomplish established goals.
  5. Knowledge of Los Angeles County Department of Mental Health (DMH) clinical paperwork required.
  6. Bilingual Spanish.
  7. Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.
  8. Physical ability to perform Pro-ACT, administer first aid/CPR and operate an agency motor vehicle.

As providers of social services, St. Anne’s employees work with clients dealing with complex emotional and mental health issues, many of whom are facing significant personal crises on a daily basis.  Employees must remain aware of any personal emotions, transference, and/or countertransference that may hinder the ability to remain professional.  As such, employees are expected to maintain composure during all interactions and crisis situations, implement a self-care plan, and utilize crisis communication to deescalate client crisis and prevent any serious bodily injury or harm.
