Case Manager at Beverly Terrace, Supportive Housing Program

Schedule:  Mon-Thurs  11am-8pm  Flex Fri  11am-7pm

JOB SUMMARY:

Working under the supervision of the Program Manager, the Case Manager will be responsible for providing intensive case management services (ICMS) in a residential setting to formerly homeless and chronically homeless families. The Case Manager will perform specialized tasks including intensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, counseling, individual benefit assistance, referrals and linkages to all tenants. The CM- will document all tenant interactions and input that data into the Homeless Management Information System (HMIS) database and (CHAMP) database. The case manager shall serve as the central point of contact and coordinator of critical services access for each of the tenants assigned to their caseload. The essential duties and responsibilities of this position include, but are not limited to, the following:

 

ESSENTIAL JOB DUTIES:

  1. Implement the Housing for Health Intensive Case Management Services (ICMS) Program Guide and apply the “Whatever It Take” philosophy to your work as an ICMS case manager.
  2. Ensure that clients are referred t and linked to needed services (i.e. medical care, mental health, substance abuse, food banks, transportation)
  3. Identify resources that a tenant needs to successfully navigate the housing process
  4. Develop, assist and facilitate weekly Life Skills and/or Informational groups that will provide tenants with support in thriving in the various Life Domains as well as their ability to maintain their housing.
  5. Provide and maintain weekly updates in CHAMP and Clarity (HMIS) by entering case notes and permanent housing updates
  6. Process housing applications and provide appropriate client outreach and engagement to achieve their housing and service goals. conduct initial biopsychosocial assessment to potential tenants for permanent supportive housing
  7. Whenever possible, conduct your meetings with clients inside their permanent housing setting (e.g., in their unit) to observe and assess how they are managing in terms of their health and safety.
  8. When and if needed, ensure you connect clients to Client Engagement and Navigation Services (CENS) and have access to the Substance Abuse and prevention Service Helpline (SASH) for substance use and prevention services
  9. Provide crisis management services to support tenant housing retention, increase income and build community engagement
  10. Initiate, monitor, and follow-up on referrals to community partners. Oversee the delivery of services for outside social service providers.  Coordinate regular meetings and evaluate progress.
  11. Responsible for reporting abuse, as a mandated reporter which includes immediately reporting any concerns with respect to violation of a person’s rights, actual and/or suspected abuse
  12. Managing and facilitating all aspects of the housing sustainability such as; lease-up agreement, re-certification process, property management advocacy, etc.
  13. Develop and maintain SMART objectives with tenants
  14. Present tenant case reviews to the Program Manager on a weekly basis

QUALIFICATIONS:

  1. Bachelor’s Degree in a Behavioral Science and a minimum two years of full-time experience working with homeless individuals or families, or equivalent work experience with this population
  2. Bilingual in Spanish/English is required
  3. Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.

  5. As providers of social services, St. Anne’s employees work with clients dealing with complex emotional and mental health issues, many of whom are facing significant personal crises on a daily basis.  Employees must remain aware of any personal emotions, transference, and/or countertransference that may hinder the ability to remain professional.  As such, employees are expected to maintain composure during all interactions and crisis situations, implement a self-care plan, and utilize crisis communication to deescalate client crisis and prevent any serious bodily injury or harm.
