Intake Coordinator

Hours Mon-Wed 6am-3pm, Thurs 9am-6pm, flex Fri 9am-5pm

JOB SUMMARY:

Maintain a healthy, therapeutic, and safe environment for all residents. Work with residents to effectively complete the intake process and become housed. Engage with alumni in aftercare services. Assist residents in developing the skills necessary for independent living.

 

ESSENTIAL JOB DUTIES:

  • Process program applicants for intake, including conducting program orientation, assisting applicants with collecting and submitting required documentation, performing assessments, and managing intake paperwork.
  • Orient new intakes to the program, including completing documentation.
  • Facilitate the intake for all eligible THP and THP+FC applicants.
  • Coordinate with LA County Coordinated Entry Systems for receipt of eligible housing referrals.
  • Collaborate with referral agencies, including DCFS/Probation, in procuring appropriate documentation.
  • Coordinate alumni program by engaging all participating alumni in follow-up aftercare services.
  • Develop and facilitate events for THP residents and alumni.
  • Assist Education Specialist in facilitation of life skills and education classes for residents.
  • When available, work with Education Specialist on any off campus college visits or events.
  • Provide crisis intervention to residents and develop safety plans to assist residents with making safe choices.
  • Operate as the Facility Manager after regular business hours.

QUALIFICATIONS:

  • Bachelor’s Degree in a Behavioral Science and a minimum two years of full-time experience working with transition-aged.
  • Bilingual in Spanish/English is preferred.
  • Good oral, written and organizational skills.
  • Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.
