Lead Case Manager

Shift: Mon-Thurs 9-6, flex Fri 9-5

JOB SUMMARY:

Under the supervision of the Residential Treatment Program Assistant Director, the Lead Case Manager oversees the intake and case management department. The Lead Case Manager develops initial treatment plans, oversees initial enrollment processes, timely completion of NSP reports, and is responsible for overall functioning of the case management team.

 

ESSENTIAL JOB DUTIES:

  • Provide direct supervision to the case management staff.
  • Oversee Intake (interviewing, assessing and creating an initial treatment plan, school enrollment, ELC)
  • Oversee the “Welcome Process” for all new residents.
  • Create and facilitate implementation plans to manage transitions within the program.
  • Oversee document collecting and timely submission of all reports (doc collection, ITP/NSPs, etc.)
  • Support case managers in developing needs and services plans (NSP) and discharge plans, and oversee accuracy.
  • Provide counseling in an effort to resolve the difficulties with resident that led to the need for placement.
  • Provide counseling to help the resident understand the reason for placement and to handle associated emotional problems.
  • Assess residents and family context to identify changing needs.
  • Maintain confidentiality as required by licensing, HIPAA, and other regulatory agencies.
  • Participate in CFTs, TTMs and PPTs.

 

QUALIFICATIONS:

  • Master’s Degree in Social Work, Counseling or Psychology is required. Registered with the BBS or ability to be registered.
  • Experience in residential treatment or work with adolescents preferred.
  • Must possess and maintain a valid California Driver’s License and a driving record.
  • Ability to perform Professional Assault Crisis Training (Pro-ACT) restraint, administer acceptable to the agency’s insurance carrier.
  • Physical first aid/CPR and operate an Agency motor vehicle.
