St. Anne’s Family Services is committed to protecting the security and privacy of our stakeholder information. On November 5, 2021, we began notifying stakeholders whose information may have been involved in a data security incident.

St. Anne’s identified and addressed a data security incident that disrupted the operations of some of its IT systems. The incident was first identified on September 3, 2021, and St. Anne’s immediately took steps to secure its systems, launched an investigation, and a third-party forensic investigator was engaged to assist. St. Anne’s also notified law enforcement.

The investigation determined that an unauthorized person accessed a network server that contained protected health information, including stakeholders names, contact information, Social Security numbers, and limited information regarding care received at St. Anne’s.  For stakeholders whose information may have been involved, St. Anne’s recommends that they review the statements they receive from their healthcare providers and contact the relevant provider immediately if they see services they did not receive. Additionally,  St. Anne’s is offering complimentary credit monitoring and identity protection services to individuals whose Social Security numbers are involved in the incident

St. Anne’s takes this incident very seriously and sincerely regrets any concern this may cause. To help prevent something like this from happening again, St. Anne’s has implemented additional safeguards and technical security measures to further protect and monitor its systems.
