Case Manager in Residential Program

Schedule: Mon-Thurs 10am-7pm, flex Fri 10am-6pm

Pay rate: $22.00 Hourly

JOB SUMMARY:

Responsible for providing services to meet the resident educational, mental health, and medical needs. Responsible for the case management and communication with county social workers regarding residents’ progress. Responsible for providing a healthy, therapeutic, and safe environment for residents and their children through direct care. Responsible to work with county social workers and probation officers to successfully transition residents into lower level of care.

ESSENTIAL JOB DUTIES:

  • Develops, creates, and implements goals and objectives of residents documented on the Needs and Services Plan.
  • Documents resident’s progress on Needs and Services Plan
  • Develop a self-sufficiency plan with each resident that describes objectives, action steps, timeframes, and supportive services necessary to achieve the objectives.
  • Assist residents with the implementation of the self-sufficiency plan by meeting formally once a week with each resident in their home and informally as often as necessary.
  • Continuously assess residents to identify changing needs and modify their self-sufficiency plan and staff interventions as necessary and plan for discharge concurrently.
  • Appropriately document the resident’s activities and program participation
  • Actively participate in weekly team meetings.
  • Active participant in resident’s Child and Family Team meetings.
  • Coordinates and manages information provided by other school professionals
  • Provides supervision, protection, and care of students individually and in groups at all times.
  • Provides assistance to each student in working with a group and in handling individual problems.
  • Administer discipline and setting of limits for behavior.
  • Documents residents progress; identifies possible need for professional services; communicates such findings to professional education staff.
  • Assist intake coordinator with intake responsibilities including screening telephone referrals, reviewing intake packets, arranging and conducting pre-placement visits and interviews, determining admission decisions, and completing all necessary admission paperwork if needed.
  • Orient new intakes to the program, including completing documentation.
  • Provide Intensive Care Coordination and Intensive Home Based Services to residents placed in STRTP.
  • Document client services in accordance with DMH standards.
  • Maintain weekly service provision requirements.

SECONDARY JOB DUTIES:

  • Participate in mandatory trainings, in-service, and meetings.
  • Participate in the Continuous and Quality Improvement (CQI) of the agency.
  • Perform other duties as assigned.

QUALIFICATIONS:

  • Bachelor’s Degree in Social Science, or related field
  • Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.
  • Physical ability to perform Pro-ACT, administer first aid/CPR and operate an agency motor vehicle.
  • One year similar experience.
  • Ability to receive and implement constructive feedback.

As such, employees are expected to maintain composure during all interactions and crisis situations, implement a self-care plan, and utilize crisis communication to deescalate client crisis and prevent any serious bodily injury or harm.

The employee is often required to stand and sit; reach with hands and arms; and stoop, kneel, crouch, or crawl.
  • The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Employee may occasionally be required to drive a motor vehicle.
